Qualifying for

If you wish to proceed with your no-cost consultation to gain a second opinion, your Patient Navigator will make the arrangements for either a face-to-face consult or telemedicine call. You’ll have the opportunity to meet with the surgeon and ask questions. If your case qualifies both financially and medically the surgery, facilitated through Hostcare Resources, would be performed at no cost or at a significantly reduced cost to you. The final decision on a surgeon and medical facility is always your choice. After your no cost consultation, simply contact your Patient Navigator to share if you wish to proceed using your Hostcare voluntary benefit.

Feel free to consult with your health plan administrator, your company health plan contact, or a Hostcare Patient Navigator with any questions about the domestic travel benefit. Talking with these contacts will help you understand the difference between your out-of-pocket cost associated with your standard medical benefit plan option (working with a local provider) versus the Hostcare medical travel benefit program.

Travel

Ground TravelIf a surgeon is located 4 hours or less by car from a member’s home, the member will be approved to drive with a travel companion.

Air TravelMembers who must travel farther than 4 hours by car for a procedure are presented with options for air travel. How long you are out of town will depend on the type of surgery. Your Patient Navigator will inform you how long you can expect to stay in the location to where are you are traveling.
